Sentinel Events
Virginia Premier Health Plan, Inc. as contracted by DMAS must maintain a system of identifying and recording a recipient's death. For each sentinel event the following must be reported to DMAS on a monthly basis.
- Recipient full name
- Recipient Medicaid/FAMIS Plus ID Number
- Recipient's PCP name
- Recipent's cause of death and the providers involved
- Date of occurrence
- Source of sentinel event data
