Sentinel Events

Virginia Premier Health Plan, Inc. as contracted by DMAS must maintain a system of identifying and recording a recipient's death. For each sentinel event the following must be reported to DMAS on a monthly basis.

  1. Recipient full name
  2. Recipient Medicaid/FAMIS Plus ID Number
  3. Recipient's PCP name
  4. Recipent's cause of death and the providers involved
  5. Date of occurrence
  6. Source of sentinel event data
