Member Services will:
- Answer your questions about Virginia Premier
- Help you change your PCP
- Explain your benefits
- Help you locate a local provider or pharmacy
- Assist in scheduling transportation services for medical needs
- Connect you with a Virginia Premier nurse
Attention members: Protected Health Information (PHI), which could identify you, should not be sent through this form. PHI includes information such as: member identification number, social security number, date of birth, and any medical information about you (i.e. medical records number, diagnosis, procedure codes). When communicating with Virginia Premier through this form, only your name and phone number should be used along with your inquiry. We will call you to obtain any additional information needed and to respond to your inquiry.
